The first two are entirely multiple choice and not easy. They can be taken in the U.K. or the U.S. After passing these, you have to pay a hefty fee to sit a third exam called the ECFMG (Educational Commission for Foreign Medical Graduates).
There is no exam called the ECFMG. The ECFMG is a certificate indicating that you have passed Step 1, Step 2 CK, and Step 2CS.

Step 3 is taken to get an unrestricted license in a U.S. state. Every state has different requirements as to when you can take the exam. Also the medical boards can also ask you to take additional exams if they feel that you're not competent enough or lack in some training. They may even require international grads to be board certified before they will grant you an unrestricted license.
