Occupational Health Form is here!!!

sorry, but i find anything from Sheffield really exciting!

but i thought your doc usually had to fill out the illnesses bit or at least sign it - do they send them an extra form or something? i know from working in my gps med students come in with like 8 page long forms about every illness under the sun?

oh well, seems simple enough

only 4 months to go! if i pass my exams next week that is

nah because then then dr can charge the occi health dept because its as good as an insurance medical. plus why amass all that paperwork when there's nothing wrong?

it was exciting though but also a bit of an inconvenience! arranging dr appts around work and then having to get things signed. why can't the practice nurse do it? feel guilty for needing him to just sign something!!! oh well.

another step closer to medicine.

ummm the practice nurse can do it. Read the print on the form and unless its changed from last year it read something along the lines of "a doctor or appropriate medical member of staff such as a practice nurse". Mine was filled out the practice nurse and i had no problems.

yes it does say GP/Practise Nurse. Just wondering what sort of things would they want more information on?? i had anxiety last year and had 4 weeks off school- will that warrant a full blown interrogation?

Hi Guys!
Yes, it's good to see the forms have finally arrived, and I must admit they're not as bad as I was expecting (still recovering from doing the student loans forms).
Not sure either Scara about the answer to your question, but I would hope it wont require too much questioning.

no the letter says that the immunisation thing must be signed by your GP. i think.
