Occupational health form

Hi I have a query about the occupatoinal health form for Notts. Do you need to disclose short sightedness under disturbances of vision, and acne under skin disorders?

i doubt it very much, unless you are seriously short sighted. (I am and i didn't mention it, i just wear contacts)

As for the acne unless it seriously impedes your working life i don't think it is important...

Likewise in attendances to hospital i don't thinkthey would be interested in my stitches from the ages of about 8 to 14 either
