I agree with Gizmo that "student doctor" is usually best (though maybe not for a reason of not looking like a wanker!?! hmmm...) but I also call myself medical student or one of the other 'options'.
Just as you might vary your address to patients (eg some prefer to be called by their first name, some prefer to be called Mr/Mrs), what I introduced myself as would depend on the patient and what I thought they understood by the term. As long as you give the patient a chance to ask questions, or explain more if they dont understand.
I think it can be really confusing for patients in hospital to understand the roles and responsibilities of the different levels of doctor... medical students, JHOs, SHOs Regs, Consultants. The different levels of nursing staff from HCAs, nurses, senior nurses, nurse practitioners etc also make everything confusing. Many assume that a medical student will know all about their condition and many also think of students in general and think of partying and drinking and will be reluctant to talk. Also when working as an HCA i was often mistaken as having the same role as a nurse.
As long as you make sure you address any misconseptions a patient has about your role, call yourself medical student/mature student/student doc whatever you feel is best. There really isnt a "best one"! I try to see myself as "me" rather than any one particular doctor label!

"Student Doctor" in clinics. The number of times my partner and I have been introduced as "med students" and not allowed in on consultations is incredible. Patients have even asked us the classic 'So, what do you want to do after university then? Hospital administration?' or the ever cringe-worthy 'You're best off being a doctor like this lot, they're rolling in it. So many people leaving university with degrees nowadays... you want to get yourself on a doctor course, be on the front line"
And I can't even begin to think how many times I've wanted to answer 'You're a student? What do you study?' with a blunt 'Fashion Studies'

ay there wuz this old lass on our ward who wuz convinced i wuz a doctor.......hehehe..... i told her a million times and all but all she said in reply wuz 'fank you for telling me, doctor.'

also, ma dad (who is a retired fruit shop/convenience store runner) gets mistaken for a doc when he goes to the 'ospital cos he wears his suit at cardiac appointments (he only looks little and all, like me hehehe).....good fing he neva walks past the ER room then!
