Hi Fizz
I actually graduated last year and I felt the same - that my uni would just say 'Oh pull yourself together!' especially as my dissertation was a mess - in the end they were ok about it. My tutors were all very supportive and they gave me extenuating circumstances consideration for my dissertation and finals. I ended up with a 2:1 which most people think is great (I'm not dissing it by any means but I was on track for a first before I got ill! Bitter... lol).
However, when considering me for finals they put me in a small room so I could get up and walk (I had back pain and couldn't sit for more than about 20minutes at a time) BUT they didn't give me any extra time to get up and walk in. So my suggestion to you is not to be afraid of asking for what you need. The worst they can do is say no and as it is a genuine medical condition if there's anything they can do to make you more comfortable then be explicit in what it is and they should provide it.
I've got to say I really really feel for you because nobody understands what it is like to be ill and not be able to do what you want unless they have been there. I hope you have lots of support at home to help you deal with everything.
I would have PM'd you but I actually have no idea how. Anyone like to clue me in?
