No don't worry. there's no additional form. just the UCAS form. if u get an interview they send u lodas of forms 2 fill in at the same time. relax and good luck

Thought this might be useful... it's the scoring allocations for UCAS forms used by Manchester for medicine.

Academic ability - 20 marks
Determination - 20
Reasons for choosing medicine - 20
Work experience -10
Intepretation of experience - 10
Hobbies - 5
Communicatio - 5

Adds up to 100. In previous years you've needed a score in the high 70s for an interview.
