Occupational Health Form

hi, has every1 got a occupational health form from leicester uni 2 fill in??
it asks you if u have immunisation for, Tetanus, Polio, BCG, Diptheria, MMR. it also asks for the date u got these jabs....i have no idea wen i took these jabs?? what should i do??? see my GP and ask him wen i took these jabs???

We were sent a pink card that the GP had to fill out and stamp with their clinic stamp. Perhaps they have changed things however since then. But you should see your GP/ Practice Nurse if you do not what vax you have had or when, as your medical records usually have this on file.

Hi guys

The occy health form is new this year. Last year, what I did to make my life easy was write a list of everything I needed to get in terms of blood tests, vaccinations, immunisations and certificates. I went along to the practice nurse (they have more time than the GP and are better with needles) and went through everything until I had ticked off everything. This takes months and months so start NOW!!

A few tips...

1. If you are a member of a 'paper - less' practice that only keeps computer records make sure that you specifically request that your lab reports aren't shredded as otherwise it's a stress to get a copy.

2. The nurse will probably tell you that there is no such blood test for checking chicken pox immunity - THERE IS! It's ablood test that not all labs carry out so it may have to be sent off to a specialised centre to be tested. Just keep asking and your GP will know.

3. If you are able to obtain a copy / computer printout of your immunisation record it's v helpful, but make sure it has the practice stamp on it and is signed so that they know it's for real.

4. Photocopy all your blood test reports / lab certificates / imms records as they lost some of mine and I ended up turning into a pin cushion all over again!!

5. Start getting everything done now if you haven't already started. It can take a long time to get Hep B protection as it has a poor uptake apparently.

do u need 2 do all the blood checks?? chicken pox, bcg n etc?? i just thought it was only Hep B.

No, afraid Hep B alone is not adequate. Varicella zoster (chicken pox) and rubella (German measels) antibody checks are essential. If you have not had an MMR course you will need to have two IM injection doses of that too, after you start, and a Heaf test for TB as well if there are no medical records or physical evidence (scar) of you having received a BCG vax.

maybe i missed this on the letter that they sent but im pretty sure they didnt mention anything about the above :S

to answer the above post.....they didnt explicitly state there is a deadline, just that it needs to be sent in as soon as poss.
