Occupational Health Form

Been given an offer for Kings and have to fill out the occupational health form. Just wondering if other people who also have it could help me out. When it asks at the top
"Have you ever visitied a GP/ been examined?"
Did you put every time you have been to the GP because I have been to the GP but I can't even remember some of the times I went, nevermind the dates. If you have put them down, did you put brief explanations eg. for migraines, or more indepth info.
The other question that is getting to me is whether you have taken any days off. I've taken the odd morning or afternoon off for migraines etc. but not full days. It asks for dates.....I have absolutely no idea, I can't remember. Is it imperative you include every single piece of information?
Would appreciate some input from other people in the same position.

hey there,

Sorry i can't answer your questions, i have the form but have not looked at it. I was under the impression I needed to do that once I had decided to accept or not - is that the case?? Have you decided to accept Kings? Alternatively, do i need to fill it in anyway (and if so, why? - do they check your health status before you can accept?...) confused!

Smile Occupational Health Form

After I filled mine out honestly I later asked a working doctor (nameless) what his advice would be to people. He said 'lie'. It seems that whlist OH physicians may mean well, once given information that suggests even minimal problems they may try to block rather than work with a young student.

My own advice would, of course, be to 'tell the truth and shame the devil' but it does seem that the chap mentioned above has gone on to a very successful career. My own OH form didn't prevent me getting in but there seems little point giving ammunition to those who may not have your best interests at heart. Good luck anyway - a migraine shouldn't affect things I hope.

Thankyou for that, the only thing I really have to say is the migraines, the other trips to the doctor are for things like excema (spelling??!!) etc etc. It just seems so picky that they want specific dates and all the ins and outs!
In answer to Meg01, I'm going to fill it out and send it back once I've accepted the offer, I just need to wait to hear from Queen Mary before I can.

Hey there. ima GKT now and had to do the occupational health form thing last year. Its pretty much useless. All they really want is to know you've been vaccinated for everything and that there isnt anything seriously wrong with you. Plus they give you a blood test and everything when you arrive so its pointless if you ask me. Anyway hope that helped. If your coming in '06 then i will be in your year as am FCNS at the moment. Anyway good luck, look forward to showing you the ropes etc. in september!
