Immunisation form help anyone?

Hey all you other freshers, has anyone had to send an immunisation form back???
Long story but basically didn't send the occupational health form back til quite recently (tbf it didn't give a deadline), and then got an email from occupational health saying please fill in attached form and return ASAP. Downloaded the form and it asks for your immunisation history, but it also says in bold to bring it with you to registration, wheras the email said return ASAP. Really confused cos I haven't seen this form before, but there was a very similar one sent with the welcome pack (on blue card), which asked about health issues and immunisations, but again said in bold do not post back but bring along to registration. I rung occupational health but and spoke to a different women, who said to e-mail back my concerns.

No. It asks for blood test results for chicken pox and reubella, which I dont have. They said I could have my hep B blood test done in Bristol as well, so i can't fill that in either. Not panicking or anything, just wondering if anyone else has been sent it or not.

the attachment is the same as the copy they sent you by post, so it will say bring it with you, don't worry. Just edit that, and send it back by email so they have an idea of how far you've gotten in2 ur imms.

then take your paper copy with you to registration as proof/updated copy.

When u go to registration, you should have the blue card for the students health service, and the papers version (like the one they sent in the email) for the faculty of Medicine and dentistry (occupational health)

hope that aint too confusing!!!
