I think your best bet would be to finish year 2 in London, and then look at transferring to a non-London medical school for clinical medicine. Obviously medics transfer to London from Oxbridge, but I know it also works the other way. For instance, I know of a person who went from King's to Oxford at the end of year 2 (she had always planned to do clinical medicine in Oxford). I also know of someone who went from King's to Southampton, this time because she had a young family and wanted to move closer to home. I think your reasons are pretty valid reasons for transferring and I'm sure you could argue a case. Why don't you email some of the medical schools you would consider transferring to and see what they say?
