The referee has to be someone who can write an academic reference... so I think you could send an application in independently and get one of your teachers to write the reference.

I wouldn't recommend it though, since you are likely to get people's backs up doing that. Obviously, I don't know your head of year, but I very much doubt that she's an ogre - I suspect that she has your best interests at heart and just doesn't feel you're working hard enough.

I think it certainly couldn't hurt to talk to her, letting her know how strongly you feel about medicine, and how you're doing academically. The 'not working consistently' stuff makes me think that she thinks you're not serious enough about your work. Ask her what you need to do to demonstrate that you're serious about your work and your medicine application.
